Registration is a simple process. All the paperwork needed to register you as a new patient to the Practice can be downloaded from the website.
Please ensure that you have completed all of the following paperwork and brought with you to the Surgery all necessary identification documents:
- Completed HS200 or HS22x Registration form (the form completed will depend on whether or not you are already registered with a GP practice within the UK).
- New Patient Health Questionnaire
- Proof of identity i.e. Passport or Driving Licence
- Proof of address i.e. a utility bill
